Report 2017-119: California State University: It Has Not Provided Adequate Oversight of the Safety of Employees and Students Who Work With Hazardous Materials (Release Date: April 2018)

Once it has developed the health and safety reporting template and campuses have used it to submit their reports, the Chancellor's Office should develop recommendations for improving campus health and safety and follow up on the campuses' implementation of any corrective actions related to these recommendations.

Annual Follow-Up Agency Response From October 2021

Systemwide Risk Management received and evaluated the information in the 2020 annual reports and made recommendations to the campuses.

The Chancellor's Office follows up on the campuses' implementation of corrective actions via the annual reporting process. In addition, a report was presented to the Chief Administrators and Business Officers affinity group in June 2021 and administrators were asked to follow up on the campus-specific findings and recommendations.

Annual Follow-Up Agency Response From October 2020

Campuses submitted their first reports in late 2019. The responses were assessed by Systemwide Risk Management at the Chancellor's Office and results were presented to Environmental Health and Safety Directors Affinity Group in January 2020. Recommendations for improving campus health and safety were included in the presentation. Systemwide Risk Management will follow up with campuses to evaluate implementation of any corrective actions related to these recommendations once they receive the responses to the 2020 annual reports, which are due in November 2020.

This recommendation requires that a new reporting template be in use before we can begin the implementation. The Chancellor's Office developed and distributed a uniform health and safety reporting template to campus environmental health and safety directors. Campuses will begin submitting reports using the new template (or their own report format as long as the reports include the required data) in November 2019. Once those reports are received by the Chancellor's Office, we can begin implementation of this recommendation.

This recommendation requires that a new reporting template be in use before we can begin the implementation. The Chancellor's Office developed and distributed a uniform health and safety reporting template to campus environmental health and safety directors. Campuses will begin submitting reports using the new template (or their own report format as long as the reports include the required data) in November 2019. Once those reports are received by the Chancellor's Office, we can begin implementation of this recommendation. In addition, a new Systemwide Environmental Health and Safety Manager started work at the Chancellor's Office on December 17, 2018; his duties include consulting with the campuses as well as conducting follow up on compliance issues.

6-Month Agency Response

This recommendation requires that a new reporting template be in use before we can begin the implementation. The Chancellor's Office anticipates that the reporting template will be implemented for fiscal year 2018-19, with campuses submitting those reports in September 2019. Once those reports have been received by the Chancellor's Office, we can begin implementation of this recommendation. In addition, the Chancellor's Office is in the final stages of recruiting a Systemwide EH&S Manager, whose duties will include consulting with the campuses as well as conducting follow up on compliance issues. The finalists were interviewed on October 23, 2018.

This recommendation requires that a new reporting template be in use before we can begin the implementation. The Chancellor's Office anticipates that the reporting template will be implemented for fiscal year 2018-19, with campuses submitting those reports in September 2019. Once those reports have been received by the Chancellor's Office, we can begin implementation of this recommendation.
